初め is to do with the first time or the start of a period of time (時間に関する)
e.g. 初めまして
  初めの日

始め is to do with starting an activity etc. (物事に関する)
e.g. 勉強(べんきょう)を始める

You can also use
べき (you should do that ⇒ ought to)  and
はず(it should be so ⇒I would expect that etc.) 
to mean should in different situations

You are right!... they write it with a line through so it doesn't get confused with a 2

You may notice some Japanese people always write their Zs like that even if not in combination with numbers.

This way of saying should is less about telling someone what to do and more about " this is how it (the situation etc.) should be"

You would not use this in spoken word, only written (as I said it is used for regulations etc.)

Yep-I'd agree with that notion

It also look like a fancy cursive "Z".
My Grandma used to write her Zs kinda like that
