OK. So I've asked some University students and some Japanese people and to be honest no-one can help.
I'm trying to find the best Japanese translation for "Epic Fail" obviously a literal translation may not be the best as the term in English has taken on quite a vast meaning thanks to the internet.

For instance you say it whenever you lose something or do something wrong or fail at something. But Obviously the word 'Epic' is vastly over the top, but still.. thats the phrase.

So far the best people can come up with is:

エピック しくじる

But I'd like to avoid using Katakana if possible.

Any one able to give it a go?

It's for a T-Shirt logo to go with a sad face. So I'm afraid there isn't anymore context to go with it.

大失敗 (だいしっぱい) sounds good to me, or how about 「歴史に残る大失敗」(れきし に のこる だいしっぱい) " a huge failure that will go down in history".

I actually never had it myself, but my friends (jp and foreign) all said it was the worst they they'd ever drank. Worse than the original square-soft potions (the second ones were ok) and worse than curry ramune. ;)
