I need help with a little something

Hi!

I have to make a survey as an assignment for my Japanese class. I've done the survey and I wonder if someone would like to read through the questions and answers and see if it's understandable ^^;;
It's not a long or extensive survey. It's very basic. It starts with a short introduction followed by six questions.
