This is trickier than you might think, because the circumstances matter a LOT. Who is apologizing to whom? What are the respective statuses of speaker and listener? What is the situation.

Some possibilities:

#1. Watashi no kashitsu deshita. Sumimasen.

A man might use "Boku" instead of "Watashi", but then, if he is speaking to a superior, "Watashi" is quite polite and might be the better choice.

For "I'm sorry", gomen nasai is less polite than sumimasen (even though it looks as if it ought to be more polite.) Mooshi wake arimasen is much more polite.

#2. Sonna koto ha, kesshite kurikaesumai.

Undoubtedly there are other possibilities. In fact, there may well be a "stock phrase", but I couldn't find it in my "stock phrase book"

#3. If anime is any guide, one shouts, "Yurusenaaaaaai!"

Seriously, "I won't forget" is easy: kesshite wasurenai (zo). It's the "what you did to me" part that I can't figure out. What pronoun to use? What verb? (ataeru?) Is this a rareru opportunity? (Oreni ataerareta zai wo kesshite wasurenai zo!...?)

It's hard enough to do melodramatic dialog in English! <g>
