JPLT - Self-Study
I'm taking the JPLT Level 4 in this December.
I'm been studying on my own ( through books, listening to tapes, ).
I worked with a Japanese company for 4 years (few years ago ),
practise Japanese with my colleagues, and I also understand kana.
What should I write up at the column on which language school I'm from?.
Should I write "self-study - Kodansha books"?.
I'm a Chinese and Chinese characters is not really a problem to me, although many differences the way Japanese use it.

Are you sure you don't want to go for 3 kyuu? Sounds like you have enough experience/knowledge to give it a shot. I would guess if kanji isn't a big issue for you, you should concentrate on grammar.

Very few native chinese take 4 and 3級, because the lack of Kanji is more of a hinderance than a help. I would suggest going for level 2, as your knowledge of kanji should help you in parsing the reading sections, and in picking out miswritten kanji (in the kanji section).
