place of birth: 私は___に生まれました。(Watashi wa _where you were born_ ni umaremashita.)
nationality: 私は__人です。 (If you're American, you say "Watashi wa Amerikajin desu.")
where you live: 私は___に住んでいます。(Watashi wa _where you live_ ni sunde imasu.)
telephone number: 私の電話番号は___です。(Watashi no denwa bangou wa _your number_ desu.)
Do you know how numbers go in Japanese...?

Ok, well I'm not too sure on the where u were born answer cos my teacher has been teaching me the word "kuni" = country.
So, here we go
Q1: O-namae wa nan desu ka?
A1: Watashi no namae wa <insert name> desu.
Q2: Nan-sai desu ka?
A2: Watashi wa <insert age> -sai desu.
Q3: -------- Not sure on this one - haven't encountered it yet and I don't want to make a guess.
Q4: The really hilarious thing about this is I can't remember :/. I know the answer..but not the question...hmmm.
Q5: -------- Dunno :/.
Q6: anata no denwa bangou wa nanban desu ka?
A6: Ok, now after each part of your number you have to say "no" like "-" = "no" So, for example, my number is 2034-155-1022.
So, you would say Watashi no denwa bangou wa ni(2) zero(0) san(3) yon(4) No(-) Ich/Ichi(1) Go(5) Go(5) No(-) Ich/Ichi(1)
Zero(0) Ni(2) Ni(2) ban desu

(The second set is obscure... I'm not sure about the usage of "de" instead of "ni" there. And I think you're trying to say "tokoro" [place])
